As you complete our enrollment process, please note we do require the following additional documentation:

• • A copy of the student's Birth Certificate
  • Proof of residence ie) Copy of driver's license/State ID or utility bill with residential address listed.
  • Copy of most current court documents pertaining to custody of the student (if applicable)

To begin the enrollment process, please complete the enrollment packet below. Once completed, return it to the corresponding office listed below. The packet can be returned via mail, in person, by email or fax.

• Technology Acceptable Use Contract
• Open Enrollment Form (This form must be filled out if your home address is not in the Annandale School District)
