Welcome and thank you for your interest in Annandale Public Schools.

To begin the enrollment process for students entering Kindergarten through grade 12, please complete the enrollment packet below. Once completed, return it to the corresponding office listed below. The packet can be returned via mail, in person, by email or fax. 

To complete our enrollment process, please note we require the following additional documentation:

• A copy of the student's Birth Certificate
• Proof of residence ie) driver's license/State ID or utility bill with residential address listed
• Copy of most current court documents pertaining to custody of the student (if applicable)

• Encuesta de idiomas de Minnesota (MN Language Survey in Spanish)
• Open Enrollment Form (Must complete if your home address is not in the Annandale School District)
